Nutrition of seniors with Alzheimer's dementia
STAŇKOVÁ, Simona
The aim of the bachelor thesis is to evaluate, whether the nutrition state of the patients suffering Alzheimer's Disease is influenced by care provided. Using the qualitative research method, five patients suffering Alzheimer's disease from nursing home Velke Mezirici were compared to five patients suffering the same disease being provided home care. Semi-structured dialogues with nurses in nursing homes were used as well as dialogues with the nurses providing homecare. The aim was to elicit the level of patients' self-sufficiency, the chance of malnourishment and caregivers' burden. Another part was one-week recording of the food and liquids intake, which was consequently put in the NUTRISERVIS application to find out the amount of energy, nutrients and liquids intaking. Based on the BMI, the energy value and nutrients need were counted by every patient and the results were later compared to the results provided by NUTRISERVIS application. The research proves that the quality of the menus is usually better in nursing homes, which is caused by the nutrition specialist and planning the menus in advance as well as checking the patients' nourishment. By patients treated in the homecare, the menus are usually monotonous and not satisfactory. The reason seems to be not enough information about the importance of balanced menus or the lack of time of caregivers. The limits according to the NUTRISERVIS application are closer to the predetermined limits in nursing home Velke Mezirici. However, both by the nursing home and by the homecare there is a problem with the intake of both fluids and fiber. Based on the interviews done, it is obvious that most the participants do not take the link between Alzheimer's Disease and the nourishment into account. Having the menus regularly checked, however, is necessary by all the patients. What is more, homecare is preferred by the most of the patients that were asked. The thesis should be beneficial for homecare patients' caretakers. At the same time, it proves that the correct nourishment and menu making might have a positive effect on patients' well-being. The results provided should be taken into consideration by the nursing homes as well.

Psychic and physical factors in a nurse profession at home care
IMRAMOVSKÁ, Simona
Homecare is given to people in their natural social environment. It is aimed mainly toward sustaining and supporting health, restoring health and improving possibility of self-care, giving palliative care to incurably ill patients and enabling peaceful dying and death. For the nurses to be able to give quality nursing care they need to be in psychic and physical balance. It is important for the nurses to be aware of the consequences of such imbalance and also of the way it influences the quality of their work especially when handling difficult situations (Misconiová; 1995; Paragraph nr. 96/2004 Col.; Komačeková, 2009). First aim was to map psychic and physical factors influencing the work of a homecare nurse. Second aim was to find differences in psychic and physical factors in the profession of nurses working in homecare and in hospital. This study is based on qualitative research using questioning through in-depth interview. Research was conducted in the beginning of February 2014. Methods of direct transcription and open coding were used for data analysis. Two questionnaires were added to the original research examining Psychic stress and Physical activity according to Baecke, evaluating the amount of psychic and physical stress at work respectively. The research group consisted of 6 nurses aged between 33-55 working for homecare agencies in the south bohemian region. Written agreements were obtained from the nurses prior to them being interviewed or be given the questionnaires. Five categories were identified from the analysed interviews: psychic stress, physical stress, handling of stress, consequences of stress and the differences found regarding psychic and physical factors observed in homecare versus hospital environments. Results obtained from the questionnaire estimating Psychic stress show average value at 3.5 which can be regarded as borderline between decreased and increased risk. The analysis of Physical stress questionnaire gave the average of 3.1 corresponding to medium load. Regarding the first aim of this work the results of the questionnaires and interview are showing greater psychic stress felt by the nurses in homecare environment as compared to physical load. The second aim comparing homecare and hospital environments shows less psychic than physical stress in homecare than in hospital environments. Practical output of this bachelor thesis will be a written recommendation for nurses working in homecare aimed toward improvement of their psychic and physical health in connection to their working experience.

Seniors and elderly are the current theme of this day and age, together with the question who, when and in what way is going be able to meet the needs of this growing group of population. In advanced age a person needs an increasing amount of aid in frequent everyday activities. The aim of this thesis is to map the spectrum of nursing care and the availability of care for aid dependent senior given by senior home institutions, home care agencies and general practitioner´s office in cooperation with the family. These research questions were chosen: How well are the physiological needs of seniors met by chosen care providers? How well are the psycho-social needs of seniors met by chosen care providers? How well is needed information supplied to the senior by the chosen care providers? How accessible is senior health care with the chosen care providers? Qualitative research was chosen for this thesis. The technique of in depth interview using open questions was used in order to obtain the required information. The conducted interviews were after agreement with the respondents recorded and then exactly transcribed. Data collected from individual interviews was analysed through manual coding, the method of pen and paper. The chosen research group consisted of 12 respondents aged 65 to 94. The average age was 87. The group included 8 women and 4 men. The respondents were divided according to the chosen care provider and decided degree of dependency. The choice of the respondents was intentional. Each provider group included all dependency degrees in order to maintain homogeneity. The results show that respondents with first and second degree of dependency, considering the provider, lack serious problems regarding satisfied needs and the given services are also satisfactory. Major differences have been identified for seniors with higher degrees of dependency with negative effects experienced especially by seniors using field services. The output of this work will be a publication of a research paper in journal Medicína pro praxi. (Practical medicine)
